This is a program set up for HIV+ persons and those of unknown status to openly discuss and explore their behaviors in terms of HIV transmission risk. This is a one on one program and does not require that clients make any particular changes in their behavior unless they feel ready and want to make changes.  Can be done in person or by phone. The program is no-cost and confidential in nature. Incentives are available for each session completed, up to 6 sessions.

HIV CASE MANAGEMENT

Case Managers are linked to HIV health care providers, and most are located in provider clinics. Spanish-speaking staff and access to other language interpretation are available. To access services through Partnership Project, please call the Intake Line at 503-517-3590 to schedule an intake; the intake can be done jointly for Partnership Project and Cascade AIDS Project. HIV-positive who meet eligibility requirements, call for intake, housing. Bus service: Fareless square.

9 a.m.-5 p.m. Monday-Friday.

Cascade AIDS Project

Housing for tenants with HIV+ status only (must have documentation). Helps families prevent eviction, develop housing plans and navigate the array of housing options. Provides long-term support to ensure stable, healthy living. Offers short-term help with housing-related expenses. Provides donated furnishings and assistance with moving.

Cascade Aids Project (CAP)

Counseling, supportive, transitional and permanent housing programs, care services for HIV positive persons and their families. Programs designed for adults and children. Workshops and classes for developing self-sufficiency.
