Accepting OHP, Medicare, and sliding fee scales for self-pay/uninsured.

8:00 am - 6:00 pm Mon, Tue, Fri; 8:00 am - 12:00pm and 1:45 - 6:00 pm Wed; 8:00 am - 5:00 pm Thur.
